Farm Malignant Hearts fast in Diablo 4 by chaining Helltides, Malignant Tunnels and Legion Events, then burn them on Varshan for huge loot spikes and smooth pushes into high Pit tiers.

If you have been hanging out in Diablo 4’s endgame for a while, you already know Malignant Hearts are not just another shiny drop, they are the core of pushing a build as far as it will go, a bit like how some players lean on U4GM when they want to grab game currency or items fast instead of grinding everything by hand. These hearts slot into your rings and amulet and quietly flip your character from “good enough” to “why is everything melting so fast”. You start chasing very specific types too: Vicious for raw damage, Brutal for defense that lets you stand still and cast, Devious for weird utility tricks, and those rare Wrathful hearts that feel like mini-uniques baked straight into your build.

      Why Helltides Become Home

Once you get serious about farming, you end up living in Helltides. They are messy, noisy and easily the best way to stack hearts quickly. While you roam, you want to watch for Grotesque Debtors, the elites with that green icon on the map. They drop hearts a lot more often than regular enemies, so you just ride from one to the next and suddenly your inventory is full. You also want to push the Threat meter on purpose. Let it cap, spawn those Hellborn champions and deal with the chaos, because their extra drops are usually worth the repair bill. If a Blood Maiden ritual pops up nearby, drop whatever you are doing and go there, since the waves of elites she summons can feed you more hearts in a few minutes than a normal zone does in half an hour.

      Malignant Tunnels And Targeted Farming

Sometimes you do not just want “more hearts”, you want one exact type to finish a build idea, and this is where Malignant Tunnels still matter. They are basically focused dungeons where you walk in with the right Invoker and tell the game what kind of Outgrowth you want to pop. Clear the place, crack those Outgrowths and you usually walk out with three to five hearts per run, and they are much closer to what you actually need. It is not as wild as an open Helltide, but on nights when you just want to put on some music and grind in a straight line, this loop feels really good. Mixing in Tree of Whispers tasks helps as well, since the Grim Favors you are doing anyway turn into caches that drop body parts, topping up your stash with basically no extra thought.

      Turning Hearts Into Varshan Kills

When your stash tabs start to look ridiculous, it is time to stop hoarding and start burning those parts at the Malignant Burrow in Hawezar. This is where you use all the hearts and body parts you have been collecting to summon the Echo of Varshan. The base summon is simple: you feed in one of each part, tied to the four heart types, then beat Varshan down for a decent shower of gear and more resources. Once your character is strong enough, you will want to aim for the Tormented version instead. That one asks for three of each part plus Stygian Stones, but it pays you back with a loot table that can drop high-end uniques that carry your main build or make your next alt feel broken from level one.

Managing Extra Hearts And Staying Ready

You will eventually end up with duplicate hearts or ones that just do not fit any of your builds, and that is where Cormond’s Wagon comes into play. Salvaging extra hearts for Ichor keeps your inventory from turning into a mess and gives you another way to chase specific effects without waiting on a lucky drop, especially if you like planning out builds for new characters ahead of time and lining up the right powers just like some players plan out which Diablo 4 iteams they want to buy before a new season hits.
